IndyCar officials have acknowledged that a malfunctioning hose complicated efforts by Texas Motor Speedway's safety workers to extinguish the fire on Simona de Silvestro's car Saturday night.

The workers eventually pulled De Silvestro out of her Dallara in flames, but the young Swiss driver burned her right hand.

League officials have since issued a statement saying workers were forced to use backup bottles to put out the fire and that it will investigate why the original equipment did not work.

The statement went on to say that all equipment is checked before each race.
